  • Date: Fill in the actual date for each day of the challenge.

  • Completed: Mark with a check (✓) if you completed your task for that day, or a cross (✗) if not.

  • Mood: Rate your mood from 1 (low) to 5 (excellent).

  • Notes/Reflections: Write a quick sentence or thought about the day’s experience, challenges, or wins.
