  • Is your environment helping or hindering this habit?

  • Are there any distractions or obstacles you can remove?


5. Emotional Check-In

  • How do you feel before doing the habit?

  • How do you feel after doing it?

  • Has this habit affected your mental or emotional well-being?


6. Outcomes & Impact

  • Have you noticed any changes in your life because of this habit (physical, emotional, social, productivity)?

  • What benefits are you seeing, and are they increasing over time?


7. Challenges & Barriers

  • What challenges have come up in maintaining this habit?

  • How have you overcome these challenges, if at all?

  • Are there any recurring issues you haven’t resolved?


8. Adjustments & Optimization

  • What’s working well that you should continue?

  • What should you stop doing or change?

  • Can you make the habit easier, more enjoyable, or more rewarding?


9. Reinforcement & Accountability

  • Do you have any accountability (friend, tracker, app)?

  • What rewards or positive reinforcement do you use?

  • Would a change in support system help?


10. Forward Planning

  • What’s your goal for this habit next week/month?

  • Will you keep this habit as it is, improve it, or replace it?

  • What’s one small action you can take today to strengthen this habit?


Optional Reflection Summary

You can summarize your responses in a few sentences:
